New Student
For a student who has not attended Leupp Schools before, in any grade from Kindergarten through Grade 12.

Gather documents

Return it
Bring the completed form and documents to the Front Office, or mail them to the address below.
Hear back
The Front Office confirms your paperwork, arranges placement, and tells you what to expect on the first day.
What to bring
Have these ready when you return the form.
- Birth certificateA certified copy for the student.
- Immunization recordsCurrent and up to date.
- Proof of addressA utility bill or similar showing where the student lives.
- Certificate of Indian Blood (CIB)If applicable.
- Most recent report card or transcriptFor students transferring in.
- IEP or 504 planIf your student has one, so services continue without a gap.
- Guardianship paperworkIf the person enrolling is not the parent.
